New patients are always welcome.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.

Consultations are available by appointment.


Office Hours


Emergencies

This web site is not for emergencies. If you have an emergency, go to the emergency room or dial 911.


Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.

Dr. Ferraro is an out of network provider for all insurance carriers.


Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

A driver is available for all patients to be brought to and from the Mahwah Surgical Center.
